IMPIEGO DELLA LASERTERAPIA DURANTE LA GRAVIDANZA: UNA REVIEW SISTEMATICA DELLA MATERNITA' E DEGLI EFFETTI SUL FETO. REPORT DAL 1960 AL 2017
I trattamenti laser NON vengono praticati in gravidanza e i trattamenti elettivi vengono posticipati post partum.
La gravidanza viene comunemente considerata una controindicazione all’utilizzo di trattamenti laser sebbene non ci siano dati scientifici che supportino in modo chiaro e definitivo questa evidenza. In caso di reale necessità la terapia viene effettuata preferibilmente nel II-III trimestre, dopo la fine dell’organogenesi fetale.
Questa Review ha preso in esame tutti gli articoli pubblicati dal 1960 al 2017 riguardanti i l’utilizzo di laser cutaneo in gravidanza.
380 donne ( 22 studi pubblicati) si sono sottoposte a trattamento laser durante la gravidanza per cause non procrastinabili : adenocarcinoma cervicale, urolitiasi, condilomi, carcinoma cervicale in situ, cicatrici cutanee, carcinoma verrucoso, Tumore di Buschke-lowenstein e acne volgare.
Sono stati utilizzati laser con diverse lunghezze d’onda in tutti i trimestri di gravidanza
In tutti gli studi esaminati non si evidenzia nessuna correlazione tra l’utilizzo del laser e l’ aborto spontaneo, malformazioni fetali e parto pretermine. In un solo studio si è verificata una rottura prematura delle membrane 4 gg dopo la laser terapia. Difficile verificare il reale nesso causa-effetto.
Questo articolo per la prima volta sostiene come NON dannoso l’utilizzo del laser durante tutta la gestazione. Tuttavia i dati disponibili sono scarsi e mancano studi prospettici randomizzati caso-controllo.

Utilization of Laser Therapy During Pregnancy: A Systematic Review of the Maternal and Fetal Effects Reported From 1960 to 2017

BACKGROUND:
Due to concerns regarding maternal and fetal safety and the absence of evidence to the contrary, laser treatment during pregnancy has traditionally been limited to situations of absolute necessity.
OBJECTIVE:
This review seeks to examine the available evidence to determine the safety of laser therapy during pregnancy.
METHODS:
Medical databases were searched for relevant reports from all specialties regarding the use of lasers during pregnancy from 1960 to 2017. A legal case review was also performed.
RESULTS:
Twenty-two publications in the literature reported the use of various laser wavelengths in 380 pregnant women during all trimesters. Other than 1 case of premature rupture of membranes questionably related to the laser treatment, there were no cases of maternal or fetal morbidity or mortality, premature labor, or identifiable fetal stress.
CONCLUSION:
The available evidence, limited to low evidence level case reports and series, indicates cutaneous laser treatment during pregnancy is safe for both mother and fetus. Furthermore, laser physics and optics dictate there should theoretically be no risk of fetal laser exposure from commonly used cutaneous lasers.
